We are recruiting a Prescribing Pharmacist for a fully remote role supporting an innovative Hospital-at-Home service. This service enables patients to safely receive care and monitoring in their own homes through digital healthcare solutions, improving outcomes while reducing hospital admissions.
About the Role
As a Remote Prescribing Pharmacist, you will conduct telephone-based medication reviews with patients enrolled in the service. You will focus on optimising medication regimens, identifying issues early, and making prescribing decisions where clinically appropriate.
This is a temporary, ongoing position, with 40 hours available per week.
Key Responsibilities
Conducting structured medication reviews with patients over the phone
Reviewing current medications for safety, effectiveness, and adherence
Calling patients to discuss symptoms, side effects, and treatment outcomes
Amending, initiating, or stopping medications in line with clinical guidelines
Clearing and managing routine medication-related alerts
Identifying red flags and escalating concerns to nurses or clinicians where required
Documenting all consultations and changes accurately on clinical systems
Providing clear advice, reassurance, and patient education
What We’re Looking For
Registered Pharmacist with Independent Prescribing qualification
Experience conducting medication reviews (primary care, community, or remote settings)
Strong clinical decision-making and risk assessment skills
Confident telephone consultation manner
Ability to work autonomously within a remote clinical environment
Excellent documentation, organisation, and communication skills
Comfortable using digital systems following training
